For your convenience here is a Mass Card order form. Simply fill out the form, then send your check for payment ($10.00 each or 3 for $25.00) payable to Office of Development and mail to DHS Office of Development 72 Church Street Putnam, CT 06260. You will receive your cards by return mail.
